Helping your patients receive treatment with CareCredit is easy. Here are some tips to maximize your approval.

Important Tip for Success:
When listing household income, include all forms of income, including child support, alimony, or
separate maintenance income, all full-time and part-time jobs, bonuses, commissions and investments.

Ask the Right Questions:

Prior to starting the application, make sure you have all of the following (this will help you save time).

Do you have a valid social security number?

Are you over 18 years of age?

Do you have a valid primary ID?
(Options include: state-issued drivers license (preferred), government-issued ID, state-issued ID,
passport, military ID or government-issued Green/Resident Alien card.)

Do you have a valid second form of ID?
(Options include:Visa,MasterCard, American Express, Discover, department store or oil company
credit card with an expiration date.)

Do you have a physical street address?
(If they use a P.O. Box, they must also provide
a physical street address.)

If you answered “YES” to all of the questions and followed the important tip for success
but failed to qualify for CareCredit, or if you answered “NO” to any of the above questions,
simply reapply with a co-applicant.

Applying with a Co-Applicant:

It’s easy. Begin a new application and have a spouse, parent, other family member, or friend sign
with the patient on the account. The co-applicant will also need to answer the above questions.
Their income, debts, and credit history will also be considered for credit approval and help
maximize your approval rates. And they will also be able to take advantage of CareCredit.
